NE Ohio Opioid Education Series | The Primary Practice Physician Program for Chronic Pain (4PCP): A Biobehavioral Revolution in Care
Managing chronic pain is one of the most significant challenges in primary care, often described as a "heart-sink" experience that contributes to practitioner burnout. Through the support of our CDC Overdose Data to Action (OD2A) LOCAL grant, WellLink Health Alliance is proud to present a transformative educational session centered on The Primary Practice Physician Program for Chronic Pain (4PCP).
Featuring Dr. Jeffrey Janata from University Hospitals and Dr. Al Musa, this session provides a comprehensive overview of the © 4PCP framework—a novel approach designed to revolutionize care by shifting from a bio pharmacological focus to a biobehavioral foundation. Dr. Janata will demonstrate how to treat the most challenging patients more effectively by reconceptualizing chronic pain as a brain disorder, allowing providers to improve outcomes without increasing their panel size.
Learning Objectives
By attending this session, participants will be able to:
· Implement the © 4PCP Framework: Gain a high-level overview of the evidence-based "toolbox" strategies that prioritize patient-centered and team-centered care.
· Optimize Clinical Efficiency: Explore data showing how this approach can improve practitioner satisfaction while cutting average visit times nearly in half, from 19 to 11 minutes.
· Support Dual-Diagnosis Clients: Engage in an interactive skill-building component focused on the clinician’s side of managing the "2-diagnosis" client (chronic pain and substance use), including de-escalation and pain-informed communication.
· Improve Patient Outcomes: Identify methods to achieve significant patient improvements across five key domains: sensory pain, affective pain, depression, fatigue, and tension.
· Navigate Readiness for Change: Utilize the Stages of Readiness to Change (SOC) to shape treatment plans and more effectively manage behavioral health referrals.

Jeffrey Janata, PhD Dr. Janata is the Chief of the Division of Psychology at University Hospitals Cleveland Medical Center and the Sihler Family Professor of Psychiatry at Case Western Reserve University School of Medicine. A Medical Psychologist with over 38 years of experience, specializes in the biopsychosocial treatment of chronic pain and has dedicated his career to training physicians to recognize and address psychological distress in medical patients.
Albert Musa, MD, MHCDS, FACP, has been a practicing general internist since 1987, primarily with the SSM Health Dean Medical Group. Throughout his career, he has held numerous leadership positions, including serving as Board Chair for the Dean Medical Group of the SSM WI Regional Health System Board, Medical Director of the Primary Care East Region of Dean Health System, and Chief of Staff at St. Mary's Hospital-Madison. He earned his medical degree from Rush Medical College , completed residencies in family and internal medicine at UW-Madison , and holds a Master of Health Care Delivery Science from Dartmouth College alongside a Six Sigma Black Belt certification. Dedicated to advancing healthcare delivery, Dr. Musa holds a voluntary teaching appointment at the UW-Madison School of Medicine & Public Health and is actively involved in academic and community research partnerships like the Wisconsin Research and Education Network and i-Practise.
